Refer a Family – Schools & Professionals

This service is completely free for all.

KSAF’s Family Outreach Programme accepts referrals from schools, Early Help teams, social care, and other professionals working with children in Kent.

Our aim is to provide short-term, targeted support to families where attendance or home-based challenges are beginning to affect a child’s wellbeing.

✔ Who can make a referral?

Schools (attendance teams, DSLs, pastoral staff, SENCOs)
Social workers / Early Help workers
Family Liaison Officers
Youth workers
GPs or health professionals
Other recognised agencies

Submitting A Referral

Please Follow Each Step Below

Step 1

Obtain Consent From The Family

Please download the Information Sharing Consent Form below, discuss it with the family to obtain their informed consent, then email the completed form to outreach@ksaf.uk

Step 2

Complete our Referral Form

Once you have obtained consent and emailed the completed form to outreach@ksaf.uk, please submit the referral through our online platform.

This helps us log the case, triage it correctly, and ensure the family is contacted within our stated timeframes.

Step 3

Referral Assigned to Case Manager

Once the referral and consent form have been received, our team will review the information and allocate a Case Manager based on the family’s needs, location, and level of support required.
